Thank you for your interest in Juan Diego Catholic High School. Juan Diego has an established national reputation for developing students who are well educated, highly skilled, socially conscious and destined to become successful leaders in our society. We offer an excellent academic program within a safe environment. The administration, faculty and staff of Juan Diego are dedicated to educating the mind, body and spirit of those students entrusted to our care.

We welcome new students, grades 9 through 12, each year into our high school. Our open enrollment draws students – of all faiths - living across the Wasatch Front including Davis, Utah, Salt Lake, Summit and Tooele counties.


All students seeking admission to JDCHS must complete a student application. Application requirements include:

  • Parent or guardian must complete and sign the application form.
  • A $210.00 nonrefundable fee payment must accompany this application or payment can be made electronically by following this link.
  • This application is NOT complete without a copy of your student’s most current REPORT CARD for incoming 9th grade students OR a COMPLETE high school TRANSCRIPT for incoming 10th, 11th, or 12th grade students.
  • Immunization record on Utah School Immunization Record Form is required. Out-of-state applicants may send immunizations on doctor’s form.
  • Birth Certificate is required.
  • Include a copy of any student evaluative testing listed on back page.
  • A copy of Catholic Baptismal Certificate is required, if requesting Catholic Tuition.

Educational Assessment Exam

All Incoming Students

JDCHS administers the ACCUPLACER Diagnostic tests and a Lexile exam as our educational assessment exams for all incoming students. The ACCUPLACER Diagnostic tests provide a detailed analysis of a student’s strengths and weaknesses that reflect college preparedness and academic performance.

The Educational Assessment Exams are FOR ALL NEW STUDENTS interested in attending Juan Diego for the 2017-2018, Semester 2, AND 2018-19 School year (Incoming 9th Grade, and new 10th -12th grade students) Cost is $25.00. Freshman Qualifying Exam

In addition to taking the Educational Assessment Exams, incoming freshman students who are interested in enrolling in honors-level courses for the 2018-19 school year, must participate in Freshman Qualifying Exams held Tuesday, Wednesday & Thursday, February 20, 21 & 22nd, 2018. There will be two Qualifying Exam sessions held each day from 3:30pm - 4:30pm and from 4:30pm - 5:30pm. No registration is necessary. Students should meet in the JDCHS room F113 at 3:15pm and/or 4:15pm and from there will be directed to exam rooms. Based on the results of the Educational Assessment Exams and Freshman Qualifying Exams, students will be recommended for their 9th grade courses. Student Shadow Program

Potential students may spend a school day shadowing one of our students. Our campus is yours to explore and discover. If you are interested in learning more about Juan Diego Catholic High School and would like to experience a day in the life of our students, we recommend scheduling a shadow day. You will be paired with a current student who shares interests similar to yours. You will attend their classes, meet new friends, and be introduced to faculty and staff.
